- Part# and Qty #
- Ship to address
- Method of payment: i.e.: Credit Card, Check or Money order. (Cash is not accepted)
- If paying by credit card, include the cc#, exp date and the address the bill goes to.
- Employee ID#
Your price will be system cost, plus tax and shipping.
GENERAL RESTRICTIONS:
- There is a maximum of 2 pieces purchased per part.
- Must be paid by Employee…no secondary party. (must use your own credit card)
- All orders placed through the program must be processed by the Employee Purchase basket ONLY.
